Event Scheduling Form

Please fill out this form completely, then press the submit button. We prefer that reservations be made at least two weeks in advance. The completion of this form is only a request for use of a room on the IU Kokomo campus. It is not an automatic or completed reservation. The person responsible for this request is cautioned that no advertising of the event is to be released until the reservation has been confirmed by the Events Coordinator, Tara Scott. The requestor is responsible for providing information on the room setup and/or equipment needed.

For your catering, AV and Parking needs, please email the Events Coordinator or you can call (765) 455-9449.

Note: Reservations for one time classroom needs are made through the Events Coordinator, for classroom scheduling for the semester please contact the Registrar's Office at (765) 455-9517 or email Tara Grant. Reservations for Havens Auditorium are made through the Haven's Auditorium Director at (765) 455-9362 or the Events Coordinator.

An email confirmation will be sent to the contact person listed above when the request is processed. If the room is not available, the contact person will be notified by phone or email.

Please notify the Events Coordinator immediately if the requested room is no longer needed.
