Family Leave Act Procedures

Family Leave Act procedures for Faculty Members Requesting Leave

 

This leave is available for faculty member for 1) the birth or adoption of a child or 2) the serious health condition of a qualified family member.  The faculty member must have had at least two years of continuous full-time service. (This means that visiting, adjunct, part-time, and intermittent service does not count in the two years).  Paid leave is available twice within a five year period and cannot exceed 12 weeks in duration.  Any additional leave (to finish out the semester, for example, is at no pay).

Procedures

1.       The begining point of the leave will be negotiated with the faculty member, the unit, academic affairs and HR.  IF a leave is to begin at the start of a semester, the leave officially start the week BEFORE the semester starts (as that is the report date for faculty) and run for up to 12 weeks. 

2.      The unit that houses the faculty member requesting the leave will be responsible for the submission of all the relevant paperwork.

3.      Initial submission:  Since the faculty member has been in continuous service for the two years and will be already loaded into the database, the initial paperwork consists of the Leave Form (signed by all parties) and a Pay Advice form that correctly indicates the extent of the leave.  Units processing the leave should count out the number of requested weeks and indicate the beginning and ending dates on the Leave Form and the Pay Advice Form.  

4.      After the end of the paid leave period:  A Pay Advice Form is submitted that indicates the extent of the unpaid leave period. 

5.      After the end of the unpaid leave period:  A Pay Advice Form is submitted that puts the faculty member back on the payroll.
